Hey Cara, I would suggest using Typeform. It’s very flexible, and a favorite of mine. Hope your project goes well! If I can support in any way, just let me know.

Hi Cara.
I just use microsoft forms. If you’ve already got a microsoft365 subscription its free. Might be a bit lite on the analysis bit though.
